Please print and fill out our patient health history form and bring it in for your initial orthodontic consultation.

Your initial orthodontic consultation is complimentary and will last approximately one hour. We will spend time discussing your particular orthodontic challenges and their solutions. Your appointment starts with a review of your medical/dental history, followed by digital photographs of your face and teeth, and a panoramic radiograph, if indicated.

Following this, Dr. Macdonald will then do a clinical examination. This information will help determine the best plan of treatment (braces or other) for your orthodontic needs. If treatment is indicated at this time, your treatment plan will be discussed with you in detail, including start time, appliances to be used, and approximate length of treatment.

Finally, our treatment coordinator will discuss the cost of all services, review your insurance benefits (if applicable), payment plans if needed, and schedule appointments to begin your treatment. If treatment is not indicated at this time due to dentition development, you will be appointed to our Growth Observation Program. This program is complimentary and allows us to monitor you until treatment is indicated.
